Interested candidates of Shimla Urban Area are requested to send their applications as per the following terms and conditions along with all the required documents by post to the Office of the Chief Medical Officer, Deen Dayal Upadhyay Hospital Campus.Applicants of Shimla and rural areas should submit the same before 5 pm on 23 May 2023.
All the applicants are requested to appear for the interview along with all the documents in the office of the concerned officer on the scheduled date. Applicants from Ward No. 1 to Ward 17 (NUHM area and Non-NUHM area) of Shimla Municipal Corporation area should be present for the interview on 30 May 2023 at the office of the Chief Medical Officer, Deen Dayal Upadhyay Hospital Shimla at 10 am and ward Ward No. 18 to 34 (NUHM area And applicants of Non-NUHM area) should be present for interview on 31 May 2023 at 10 am in the office of the Chief Medical Officer. in District Shimla. At the block level, the interviews of ASHA workers will be taken in the office of the concerned Block Medical Officer from 10 am on 30 May 2023. 1. The applicant must be a permanent female resident of the same ward or gram panchayat for which she wants to apply.
2. The age of the applicant should be between 25 to 45 years, and preference will be given to married/widow/divorced/abandoned women.
3. The educational qualification of the applicant should be 10th pass for urban level and 8th pass for rural level.
4. Applicants should be proficient in speaking the local language.
5. Applicants should bring the following documents with them: Ration Card, Aadhaar Card, Educational Qualification Certificate, Character Certificate, Age Certificate (10th Class School Leaving Certificate or Birth Certificate), Permanent Resident Certificate (certified by the Permanent Principal or equivalent), P.L. certificate (if applicable), SC/ST certificate (if applicable), Single Woman/Widow Certificate (if applicable), and two passport-sized photographs.
